The Compliance Form must be submitted to the Fieldwork Coordinator two (2) months before the start of your fieldwork. For students beginning in the Fall semester, the deadline is July 1st. When submitting your compliance form be sure to attach proof of liability insurance and your unoffical transcripts. All material should be faxed, emailed or dropped off to the Fieldwork Coordinator in Room 422E Thompson Hall.
