Welcome to the Office of Insurance and Immunization Records Website!
Health Services at Columbia (HSC) provides integrated and accessible services and programs that support the well-being of the campus community, and the personal and academic development of students. HSC is comprised of more than 100 individuals: medical providers, disability specialists*, nutritionists, health educators, therapists, psychiatrists, peer counselors, student personnel, support staff, and administrative professionals.
HSC also provides primary care for the insurance program. Students must be seen first for any minor injuries or illness (non-emergency) and for referrals to other physicians within the network of healthcare providers.
Health services also includes counseling and psychological services, health education, advocacy and services for students with disabilities*, support for survivors of sexual assault, as well as the public health services the department provides to the university community. *(Students with disabilities should contact the Office of Access and Services for Individuals with Disabilities (OASID) at Teachers College, 212.678.3689)
The Off-Campus Services Program** provides essential services for all students that are usually not covered under employer insurance plans. The care provided under this program may be accessed confidentially by all students without informing their insurance carrier, parent or spouse. The services provided are:
