Please complete a Vaccination Consent Form prior to your scheduled appointment and bring them with you. We will have blank forms available to you on the day of your shot as well. Forms provided below are only for the 11/14 sessions, and you may be required to complete different forms at the CU flu shot events.

Please note that there will also be flu shots available on November 10th from 2-6 PM courtesy of Columbia Health and the Office of Student Activities & Programs, on a first come-first served basis. If you are unable to attend these events at TC, please visit one of the flu fairs this fall at other Columbia University locations. You can also receive your free flu shot by scheduling an appointment with Medical Services during clinical hours.
