H-1 Visa Petition (To be Completed by Hiring Departments)

This information should be submitted to whenever you are considering an offer of professional employment to an individual who does not hold US citizenship or Permanent Residence. This information is necessary to obtain the "prevailing wage" for the position, the FIRST step in the application process for an H-1 visa. Please inform me if prospective employee is Canadian, as it may be preferable to apply for TN status. Also, please attach or forward a copy of the job posting/description from Human Resources.             The following must be forwarded to Marion Boultbee before the petition can be filed with the Department of Homeland Security (DHS):
    1. Letter from Department Chair, Project Director or Department Director
      • To:
        • Department of Homeland Security/USCIS
          California Service Center

          Laguna Niguel, CA 92677
      • Content:
        • Brief Description of Duties, Dates of Appointment (maximum 3 years), Salary, Special skills and relevant background of applicant

    2. Checks, payable to Department of Homeland Security (address above; vendor # 43-2000174):
      1. $320. filing fee (no additional fee for dependents, change of status or consular notification);
      2. $500. for the new "anti-fraud fee," unless the beneficiary is currently employed in H-1 status at Teachers College;
      3. $1000. if you need Premium Processing (contact Marion Boultbee, x3939).
