CHES/MCHES CONTINUING EDUCATION CONTACT HOUR (CECH) CREDITS AVAILABLE FOR IN-PERSON AND WEBCAST PARTICIPANTS


Teachers College, Columbia University is a designated NCHEC PROVIDER (# SEP4090) for the March 15-16, 2013 conference event--The Fifth Annual Health Disparities Conference at Teachers College (TC), Columbia University.

TC is designated for Certified Health Education Specialists (CHES) and/or Master Certified Health Education Specialists (MCHES) to receive up to a total 10 Category I continuing education contact hours. Maximum advanced-level continuing education contact hours available are 10.

Participants who attend the conference in-person or view the Webcast are eligible for Category I continuing education contact hours.

Continuing education credits will be reported to NCHEC by June 15, 2013

 

$10 COST OF CHES/MCHES CONTINUING EDUCATION CONTACT HOURS

The cost is $10. For those who attend the conference in-person, you will make your check payable to Teachers College, Columbia University and write in your memo section: CHES/MCHES fee. This check will be submitted on the morning of Friday March 15 when you register in-person for the conference, specifically, for CHES/MCHES continuing education contact hours. NOTE: You should have already registered online (at the conference website - http://www.tc.edu/healthdisparitiesconference/index.asp?Id=Registration&Info=Conference+Registration) for conference participation in advance, even as we will accept walk-in registrants; only those who register online via the above link by March 11 are guaranteed a professionally printed conference program.

 

Those who register for participation in the conference Webcast (even the day the Webcast has started, as you can watch any archived sessions that you missed) at http://www.tc.edu/healthdisparitiesconference/index.asp?Id=Registration&Info=Conference+Webcasting can mail their check to Dr. Barbara Wallace (attention Erik Santacruz), Department of Health and Behavior Studies, 525 West 120th Street, Box 114, Teachers College, Columbia University, New York, NY 10027. Only those who have paid their $10 will receive their CHES/MCHES certificate.


IN-PERSON CHES/MCHES

If you are attending the conference in-person, you will specifically register for the CHES/MCHES continuing education contact hours at the registration desk (submit your check for $10) and receive your packet of session evaluation forms, as well as the instructions for turning in all materials at the end of the conference so you can receive your certificate for 10 Category I continuing education contact hours.

WEBCAST CHES/MCHES

Once you have registered (see link from home page) for the conference Webcast, then you can  DOWNLOAD THE WORD DOCUMENT, BELOW, containing the packet of session evaluation forms. In order to receive CHES/MCHES credit you must download, complete, and return this packet of forms to Erik Santacruz, 5 Conference Administrative Assistant, at "Erik Santacruz" es3008@tc.columbia.edu. Upon return receipt of the completed forms, Erik Santacruz will send out as an e-mail attachment your certificate for 10 Category I continuing education contact hours.
