In our continuous effort to provide a safe environment for our students, the Office of Access and Services for Individuals with Disabilities would like to know if you might need any special assistance in the event of a campus emergency (ie. fire alarm, evacuation, etc.). In addition, we would like to know the type of assistance that you may require.

This information will be collected and utilized by our OASID staff, the Office of Safety and Security, and any other involved emergency personnel (ie. fire, police, EMS, etc.) in the event of an emergency.

In an effort to provide the most efficient communication concerning emergency assistance, we ask that you update us each semester of those needs for assistance you may have, either by submitting this form or sending an email to oasid@tc.edu indicating building, room, day and time.
