CHES/MCHES CONTINUING EDUCATION CONTACT HOURS

IN-PERSON AND WEBCAST

Teachers College, Columbia University is a designated National Commission for Health Education Credentialing (NCHEC) PROVIDER for this event. The conference is designated for Certified Health Education Specialists (CHES) and/or Master Certified Health Education Specialists (MCHES) to receive up to a total of 10 Category I continuing education contact hours. Maximum advanced- level continuing education contact hours available are 10. Both those participants who attend the conference in-person and view the Webcast online are eligible for the 10 Category I continuing education contact hours. Credits will be reported to NCHEC by May 5, 2015—if and only if you have submitted your Evaluation Forms for each session that you attend in person, or view online as a Webinar (see details, below).

 

PAY THE $25 FEE FOR CHES/MCHES CONTINUING EDUCATION CONTACT HOURS BY CREDIT CARD:

When you register for the conference via the conference website – www.tc.columbia.edu/healthdisparitiesconference/Registration – you will be asked to select your PURCHASE OPTIONS (e.g. Student, Professional, Community Member), which include Continuing Education Credit. Thus, make sure you choose not only your PURCHASE OPTION of Professional, for example, but also select the PURCHASE OPTION of Continuing Education Credit. The PURCHASE OPTION of Continuing Education Credit will cost an additional $25. You will be able to pay for all of your PURCHASE OPTION selections by credit card on our secure website.

 

YOU CAN RECEIVE CHES/MCHES CONTINUING EDUCATION CONTACT HOURS IN 3 WAYS:

  • FOR IN-PERSON CONFERENCE ATTENDANCE you have to turn in your Session Evaluation Forms at the end of each conference day, or at the end of the conference to the Graduate Conference Coordinator, Brett Burnham. You can obtain these forms at the Registration Table on the days of the conference. Or, you can download the Session Evaluation Forms from the website (See Link, Below—which will be a working link by/before March 1, 2015), complete them, and send them as an e-mail attachment to Brett Burnham at bmb2173@tc.columbia.edu.
    On the day of the conference, it is also possible to pay for your CHES/MCHES Continuing Education Contact Hours. You can pay by credit card online via our conference registration portal, or make checks payable to Teachers College, Columbia University and write in the memo section: CHES/MCHES fee and your CHES/MCHES number. This is part of the requisite task of officially registering for the CHES/MCHES continuing education contact hours.
  • FOR WATCHING THE WEBINAR AS A LIVE STREAM, OR FROM THE ARCHIVE PROVIDED BY TOTAL WEBCASTING, please register by clicking the link in the confirmation e-mail you receive after you register and pay by credit card on the conference website. Look out for the confirmation e-mail. Click on the link in that e-mail that takes you to the TOTAL WEBCASTING website where you have to register with them. That registration will involve you creating a PASSWORD so you can access both the main conference sessions (e.g. in Cowin Auditorium) as a WEBINAR - LIVE STREA, and/or the ARCHIVE of the main conference sessions at your convenience (e.g. the week after the conference). Here, your must download the Session Evaluation Forms from the website (See Link, Below—which will be a working link by/before March 1, 2015), complete them, and send them as an e-mail attachment to Brett Burnham at bmb2173@tc.columbia.edu.
  • COMBINE # 1 AND # 2, ABOVE, such that you attend some sessions in person (e.g. Saturday), and watch some sessions online (e.g. Friday session). Here, you must either receive your Session Evaluation Forms at the conference registration table, and/or download the Session Evaluation Forms from the website (See Link, Below—which will be a working link by/before March 1, 2015); then, complete them, and send them as ONE TOTAL/COMBINED e-mail attachment to Brett Burnham at bmb2173@tc.columbia.edu. This is essential.

Remember, to receive up to 10 CHES/MCHES Continuing Education Contact Hours, these forms must be completed and sent as an e-mail attachment to Brett Burnham at bmb2173@tc.columbia.edu. This is essential.

 

THE FOUR-STEP CHECK-LIST FOR RECEIVING CHES/MCHES CONTINUING EDUCATION CONTACT HOURS:

  1. SUBMIT THE $25 FEE BY FOLLOWING THE INSTRUCTIONS, BELOW
  2. DOWNLOAD FROM THE CONFERENCE WEBSITE THE PACKET OF CHES/MCHES SESSION EVALUATION FORMS –as a WORD FILE where you can enter your ratings and comments
  3. WATCH ALL OR AS MANY AS YOU LIKE OF THE MAIN SESSIONS FOR WHICH YOU CAN RECEIVE UP TO 10 Category I Continuing Education Contact Hours—doing so by 3/13/15
  4. SUBMIT THE EVALUATION FORMS FOR EACH SESSION VIEWED AND EVALUTED BY 3/13/15, placing them in the hand of Brett Burnham at the conference if you attend in-person, or by sending your evaluation forms as an e-mail attachment to Brett Burnham at bmb2173@tc.columbia.edu.

 

YOU MUST FOLLOW THESE 4 STEPS TO RECEIVE YOUR CHES/MCHES CERTIFICATE FOR 10 Category I CHES/MCHES sent to you via e-mail as an attachment; and, in order for NCHEC to receive notice of your receipt of the continuing education contact hours by May 5, 2015.

REMINDER: Your CHES/MCHES Continuing Education Contact Hours will be reported to NCHEC by May 5, 2015—if and only if you have submitted your Session Evaluation Forms for each session that you attend in person (place them in the hand of Brett Burnham at the Registration Table), or view online as a Webinar by sending your Session Evaluation Forms to Brett Burnham at bmb2173@tc.columbia.edu. We must also have evidence that you paid your $25 fee, as indicated in the instructions, above.
