Hours, Fees & Policies
During the regular academic year, September thru May, the Clinic is open:
Monday - Thursday from 9:00am to 5:00pm
Friday 9:00am to 3:00pm
In June, July and August, the Clinic is open:
Monday - Thursday from 9:00am to 5:00pm
Friday 9:00am to 1:00pm
Due to the increasing demand for services and its responsibility to train graduate students, the Edward D. Mysak Clinic for Communication Disorders must uphold an attendance policy:
- If a client has more than 3 absences unrelated to extraordinary circumstances, or religious holidays, the Clinic Director will review the client's attendance record to determine if services should be terminated.
- If a client arrives more than 15 minutes late for a scheduled session, the session will not be conducted and will be counted as an absence.
- Every effort will be made to make up any session missed or canceled by our clinicians. It is unlikely that sessions canceled by the client will be able to be rescheduled.
- Clients will not be reimbursed for missed or canceled sessions because fees are charged by semester not per session.
Please refer to the attendance policy for more detail:
- Parents and caregivers must remain in the Clinic throughout the therapy session.
- Per TC policies, all minors under the age of 12 must be accompanied by an adult when using the restroom.
- There is no eating or drinking allowed in the waiting room or the observation corridors.
- We ask that clients and their families refrain from using cell phones while in the Clinic.
- No video taping of the sessions is allowed.
- Diaper changing must be done in the restroom on the first floor of the Zankel Building, where an appropriate diaper changing unit is provided.
- Entry into the toy room is allowed only with an accompanying clinician.
Fees are payable to Edward D. Mysak Clinic for Communication Disorders by using our online CashNet system. Payments are not accepted at the clinic. Directions for paying your bill online will be provided with your first statement. We do not accept insurance; however, after payment is made, we will be happy to provide you with copies of documentation for you to submit to your insurance carrier.
- Fees for all therapeutic services are charged by the semester, not by the session. Fees are not refunded for absences. Please refer to the attendance policy for more detail.
- Fees for all therapeutic services will be billed at the start of each semester. Fees are due by mid-semester. Clients who have not paid their bill in full by the end of the semester will not be scheduled for further therapeutic sessions.
- The Edward D. Mysak Clinical Staff will prepare specialized reports upon request. A consultation fee of $100.00 will be billed for requests that require preparation beyond the duplication of file documents.
- A sliding fee scale is available for clients with a documented need. Please contact the Mysak Clinic for an application.
- The fee policy is subject to modifications each semester.
Commitment to Privacy
The Edward D. Mysak Clinic for Communication Disorders at Teachers College, Columbia University is committed to protecting the privacy and confidentiality of all clients receiving services. This notice describes how information about you may be used and disclosed and how you may access your information.
The Mysak Clinic is not a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). However, the clinic follows strict confidentiality policies designed to protect client information and uphold professional and ethical standards of care.
Information collected during evaluations and treatment may include personal, contact, and health-related information. The clinic maintains procedures to safeguard this information and limit access to authorized personnel.
Uses and Disclosures of Information
Information collected by the clinic may be used or disclosed for the following purposes:
Treatment
Information may be used and shared among graduate clinicians, supervising speech-language pathologists, and authorized faculty members in order to provide evaluation, therapy, and related clinical services.
Payment
Information may be used to process billing for services provided by the clinic. The Mysak Clinic does not bill insurance companies directly. Documentation may be provided to clients who wish to submit claims to their insurance carriers independently.
Health Care Operations
Information may be used for clinic administration, quality improvement, supervision of graduate clinicians, and training within the Communication Sciences and Disorders program.
Educational and Training Purposes
As a teaching clinic, sessions may be observed or recorded for educational and supervisory purposes. These recordings are used to support clinical training and improve the quality of services provided.
Appointment Communication
The clinic may contact clients regarding appointment reminders, scheduling updates, or information related to treatment services.
Legal Requirements
Information may be disclosed when required by law, including in response to court orders, legal proceedings, or public health reporting requirements.
Client Rights
Clients have certain rights regarding the information maintained by the clinic. These include the right to:
- Request access to their clinical records
- Request corrections to inaccurate or incomplete information
- Request limitations on certain uses or disclosures of information
- Request confidential methods of communication when possible
- Request a copy of the clinic’s privacy notice
Requests regarding access or corrections must be submitted in writing to the clinic.
Changes to This Notice
The Mysak Clinic reserves the right to update or revise this notice at any time. Updated versions will be posted on the clinic website and will apply to information previously collected as well as information collected in the future.
Welcome Packet & Consent
Before beginning services at the Edward D. Mysak Clinic for Communication Disorders, all clients are asked to complete the clinic intake and consent packet. This packet includes important information about clinic policies, privacy practices, consent for services, and billing procedures. It also collects basic contact and billing information needed to begin care.
Please review and complete the forms prior to your first appointment whenever possible. Your assigned graduate clinician will review the documents with you during your first session and answer any questions you may have.
Completed forms help us ensure that services can begin smoothly and that you understand the policies and procedures of the clinic.
