How to Complete Forms

Our forms are interactive PDF files that you can complete on your computer. Please use Adobe Acrobat to complete the forms. If you use your browser or Mac Preview, we will not be able to properly view or process your documents. Do not send PDFs using EchoSign.

If the interactive PDF option does not work for you, you may complete your form by hand, scan it as a regular PDF, and send your completed form via email to financialaid@tc.columbia.edu. Please include your TC ID number on all documentation that you submit to our office.

2023-2024 Loan Change Request Form: If you need to make any adjustments to your federal student loans, please use this form. The deadline to submit any federal loan change requests for a given academic term is the last day of classes of that term (Fall, Spring, and Summer). Complete the 2023-2024 Loan Change Request Form (UPDATED).

2023-2024 Federal Work-Study Waitlist Form: If you do not receive Federal Work-Study (FWS) funding as part of your initial financial aid offer, you may submit a waitlist request to be considered for FWS funding. 2023-2024 Scholarship Redistribution Form: Use the 2023-2024 Scholarship Redistribution Form (UPDATED) to change the disbursement schedule of your scholarship. You may not move scholarship funds from one academic year to another.

2023-2024 Budget Adjustment Form: Use this form to request an increase in your Cost of Attendance (COA). Please note that submission of this request does not guarantee approval. Before submitting the 2023-2024 Budget Adjustment Form (UPDATE), please make sure you attach all supporting documents with your request.

FAFSA Verification Resources

If your FAFSA has been selected for verification, please check your myTC portal to view your outstanding verification requirements, and review any email communications you received from our Office of Financial Aid. You have 30 calendar days from the date on your notice to obtain all required documents and initiate the verification process. Please contact us should you have any questions about the FAFSA verification process. In addition, you may be required to visit the Office of Financial Aid in person to provide these documents.

When you verify the accuracy of your FAFSA data, please submit all additional forms of documentation to our Office of Financial Aid to avoid delays in the packaging and disbursement of your federal aid. The verification worksheet(s) you may need are included on this page:

If you need an IRS tax return transcript for FAFSA verification purposes, please follow these steps:

1. Visit www.irs.gov and select the “Get Your Tax Record” option. You may request a transcript online or by mail. Important: Be sure to request the IRS Tax Return Transcript and not the “IRS Tax Account Transcript."

2. Once received, submit a copy to the Office of Financial Aid. Important: For security, you may redact your social security number from the document before submitting it for verification.
