In it Dr. Adrian Paraschiv (a Hospitalist and one of our Glycemic Improvement Team Members) and Lorraine Porcaro discuss how they use data and external benchmarks to identify needed quality improvements to improve glycemic outcomes at our hospital.

SHM used this specific project as a resource for other hospitals, providing an example of a QI project from identification through implementation and the results. In this project we used data to identify the need for timely reassessment of hypoglycemia, which became an important patient safety initiative. They continued to use data to track the effectiveness of interventions, and continue to include it on our monthly Glycemic Scorecard to assure sustainability of results.
