Academic Year 2018-2019 Certification Examination
The deadline for submitting the October 2018 Certification Examination form is Thursday, September 6, 2018. The deadline for submitting the February 2019 Certification Examination is from Thursday, November 1st, 2018 through Thursday, January 24, 2019. The deadline for submitting the June 2019 Certification Examination is from Friday, March 1, 2019 through Friday, May 24, 2019. Please contact the Office of Doctoral Studies if you have any questions.
Application for Advanced Seminar and Instructions (Data Hearing)

Change of Dissertation Committee Form
Change of Program Plan
This form is used if the student has taken classes that are different from the previously approved Program Plan. Upon being Recommended for Certification, students should compare their approved Program Plan with their current transcript. If there are differences (i.e., different classes taken than previously expected, new transfer credits approved, etc.) then this form must be filled out and submitted. Last Update: July 2015
Computation of Grade Deciles
This form is used primarily by the student to compute his or her grade decile to confirm he or she meets the minimum College requirements. This form is not turned into the Office of Doctoral Studies. Last Updated: August 2014

Personal Exemption/Waiver from Dissertation Advisement for Doctoral Students
This form is for doctoral students who are already Obligated for Continuous Enrollment who wish to request a Personal Exemption or Waiver.
